Gelo ACCT-A-216817 is a 8-month-old female who came in as a stray on 4/4. Medical: On 4/9 exam Doctor noted: “Patient had urine in cage this morning, and it did not appear blood tinged. II/VI intermittent parasternal systolic murmur ausculted on recheck today. Temperature 102.3 (improved from 103.9 F 4/8) On 4/8 staff noted: Temp.103.9°F On 4/7 recheck Doctor noted: “Extensive bruising noted in right inguinal region. Enlarged right inguinal LN, rest of LN palpate WNL. Abdomen - very tense and uncomfortable on palpation. Non-weight bearing on right hindlimb. Edema, pain, and decreased ROM noted on palpation of tarsus. Edema extends along limb and into right inguinal region, which is very painful.” On 4/6 exam staff noted: “Recheck for cat, unknown trauma, swelling of RH, hematuria. RH leg painful to touch, NWB, swelling and edema around the hock, extending to the toes and up to the stifle. P is resistant to extension of the limb. Abdomen soft, small bladder, not enough urine to take a sample via cysto. Based on exam and radiographs, suspect dislocation of the bones of the tarsus vs joint infection as primary cause of swelling and pain in the joint. No fractures observed. No clear cause for possible infection of the joint has been identified, but would be consistent with swelling and fever. No dislocations observed directly, but we were not able to get an ideal view of the tarsus/metatarsus on radiographs due to P being in pain. Possible causes for the hematuria include trauma to the abdomen (less likely, as no effusion or pain was noted in the abdomen) vs UTI (not able to confirm, would need a sterile sample) vs cystitis secondary to stress/pain from the R leg. Would recommend a full UA to make a better determination.” On 4/5 exam Doctor noted: “Bright, alert responsive. Could no assess mouth due to fractious nature. Non weight bearing RHL lameness with crepitus and pain near the R tarsal joint, soft tissue edema and erythema of the R tarsus. 1. Lactate: 3.0 2. CBC: WBC 37.51 (H), neutrophils 19.76 (H), lymphs 16.99 (H), eos 0.05 (L), all other values WNL 3. Chem 17: Glucose 213 (H), BUN 14 (L), Cl 113 (L), ALT 646 (H), GGT 3 (H) 4. Combo: (-) 5. 4Dx: (-) 5. XR: Salter Harris fracture of the very distal R tibia and fibula with associated significant soft tissue edema. Temperature 105.4°F Assessment: 1. SH fracture of R tibia and fibula with associated soft tissue swelling, potentially cellulitis 2. Febrile 3. Interstitial dehydration 4. Elevated ALT r/o trauma 5. Hematuria r/o trauma 6. Inflammatory leukogram r/o cellulitis” On 4/5 foster noted: “Not putting pressure on paw and possible blood in urine.” On 4/4 intake staff noted: “Was abandoned, possibly HBC. Bright, alert, responsive, with no visible injuries or reasons to suspect HBC. Pt was dehydrated with light pink gums and a slightly elevated temp. Pt was able to walk with no issues and had cuts or wounds. Fearful but allowed handling.” Behavior: On 4/7 staff noted: “Had to stop and say hi to Gelo after she meowed at me and ran over when I walked by. She's super sweet, immediately soliciting attention and worked very hard to run to greet me despite not being able to use a leg. Allowed all handling and full body pets and then began to eat, so maybe a bit of a social eater. 10/10 would recommend.” - ACCT Philly is located at 111 West Hunting Park Ave in Philadelphia.
